PicoBoo Plus scare sound issue

first of all..this is my first controller so I could very well just be doing something wrong. my ambient sound works perfect but when i trigger the scare sound all 4 outputs go off randomly and all of the scare sound doesnt play..i havent exceeded 2 minutes..not even close..i just wonder if im doing something wrong
